Procedure

• Visit the concerned authority personally.
• Fill the application form with the required information.
• Submit the necessary documents.

Required Documents

• 3 recent passport-sized photographs of the applicant
• For new application, the photocopies of the following is required:
  • MyKad
  • Certificate of registration of society (church or temple or association)
  • Proof of approval for change of name, address and constitution (if applicable)
  • The organisations constitution
  • List of current committee members of the church or temple or association
  • Current annual financial statement - for associations only
  • Certificate displaying minimum academic qualification of MCE or SPM or SPMV
  • Not older than 70 years old
  • Not a commissioner for oaths
  • The following photographs are to be affixed onto A4 paper:
    • Frontage of premises
    • Security features of the premises (windows and doors with grilles)
    • Locked steel cabinet to store controlled documents
• Application for extension (photocopies of documents are required):
  • Application for extension must be submitted to the NRDs district office at least three months before the expiry date of the current appointment
  • List of current committee members of the church or temple or association
  • Current annual financial statement - for associations only
  • Proof of approval for change of name, address and constitution (if applicable)
  • Not older than 70 years old
  • Not a commissioner for oaths

All photocopies of documents must be certified by the Registrar of Marriages and bear his signature, name, designation and official seal

Eligibility

• Must be a committee member of a church, temple or communal association handling the customary affairs of the community who is supported by the organisations committee.
• Must be voluntarily appointed and is deemed to be a public servant and subject to the Penal Code.

This service is free of charge.
