Apply In-Person:
- To apply for a Veterinary Clinic License or Permit shall be made to the Board of Veterinary Surgeons office, which falls under the Director of Animal Health and Industry Ministry of Agriculture, and Food Security Contacts
- Interested applicants are required to collect and complete the Premises Registration Form, obtainable at the Board of Veterinary Surgeons office Inspectorate Department office.
- Submit the relevant supporting documentation as mentioned under the “Required Documents” section of this page, in addition to the duly completed relevant application form(s), in all respect, to the Board of Veterinary Surgeons office Inspectorate Department office. Any omission of these documents will render the application process incomplete.
- Upon receipt of the application, the receiving officers will check for discrepancies or ineligibility in the application documents. If a discrepancy that cannot be immediately corrected exists in any of the documents, the application and all the submitted documents will be returned to the applicant and should not be re-submitted until the discrepancy has been rectified.
- If everything is in order the application will be registered for processing and consideration by the licensing board/ Committee.
- Applicants will be required to pay a prescribed fee as may be detained by the board and will be issued with a payment receipt and an acknowledgment letter, which serves as proof of submission of the application and for inquiry referencing if the need arises.
- The board/Licensing Committee for purposes of ascertaining whether the premise to be used is suitable for the purpose and will be operated in accordance with standards of good practice may request additional information relevant to the application. In such a case, the information must be submitted within the stipulated time from the date of notification. Where the applicant fails to submit the required information, the Authority board/Licensing Committee shall reject the application.
- If necessary the Authority shall schedule a premise inspection visit with the applicant to determine if the information provided is accurate.
- During the Inspection visit, any discrepancies will be brought to the attention of the applicant and corrective action must be taken before a license is issued.
- Upon successful implementation of any discrepancies noted during the inspection visit, a follow-up inspection may be conducted by the inspection team to ascertain the effectiveness of the implementation (where applicable).
- Notice of the decision (Approval, Deferral, or Rejection) for the application will be communicated to the client in writing.
- Where the board/Licensing Committee is not satisfied that an applicant meets the registration requirements, it shall reject the application and notify the applicant in writing, stating the reasons for the rejection.
- Where the Board is satisfied that an applicant meets the requirements, it shall notify the Registrar who will then draw up the license as directed by the board/licensing committee and present it to a senior officer to be certified (signed/stamped) and made available to be issued to the client.
- While collecting the license, the applicant will be required to produce a proof of identity document and an approval letter.
