- Here's how you can timely register a death in the city of Makati. First, you will need to properly fill out a Certificate of Death/Fetal Death - three (3) original copies.
- Submit the completed certificate together with the transmittal letter/slip to the City Civil Registration Office (CCRO). Your documents will be validated and will be assigned a transaction number. The transmittal form will be stamped "Received by/date/time." The receiving copy of the transmittal will be returned to you.
- Aside from the filled up certificate you will also be asked to submit a medico legal certificate, if the deceased was pronounced dead-on-arrival, has expired in transit, or in case of questionable death.
- Your application will be reviewed and validated. The city civil register will decide whether to approve or not your registration.
- Once your application is approved you will be asked to present the receiving copy of transmittal/follow up slips. You will be instructed where to get a Community Tax Certificate (CTC). In case your application is denied, you will receive a written advice on what action to take for compliance for unregistrable documents.
- Once ready, the civil register will release your copy of the civil document.
