Apply In-Person

  1. To Obtain Medical Certificate for Driver License, the applicant has to approach nearby RTO offices link, link
  2. An application form will be provide to the applicant or the applicant can download the application form from the following link link
  3. The applicant has to enter all the details in the application form.
  4. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the documents with the completed form to the Chief Medical Officer (CMO).
  7. The medical officer will examine the application and the results submits by the applicant.
  8. After successful completion of verification, the officer will issue the medical certificate.

Need for the DocumentEdit


  • A medical fitness certificate is issued by medical practitioner who is eligible to issue for the needy. This certificate assures the health condition for issuance of DL.
  • A medical fitness certificate is required to apply for a driving license in few conditions as per our Indian norms.
  • For non-commercial drivers below age 50 : self-certify one's own health by submitting Form 1.
  • For commercial drivers and non-commercial drivers above age of 50 need to submit Form 1A, filled, signed and stamped by a registered medical practitioner.
