Procedure

Apply In-Person

1. To apply for a Conductor License in Madhya Pradesh the applicant has to approach the nearest RTO office – link

3. The applicant should make sure they are eligible for applying for Conductor License by reading through the eligibility criteria that has been listed out under the “Eligibility” section of this page.
4. Please make sure you have all the documents that are listed down the “Required Document” section of this page.
5. Application forms can also be obtained from the appropriate RTO offices that can be completed and presented with appropriate fees.
6. Present the completed application forms in one of the counter and pay the appropriate fee where the officials would start processing your application.
7. You have to submit a medical certificate of fitness along with a certificate showing competence to undertake first aid work in Form F.A.B.
8. Form No. 1 & 1A (Application-cum-Declaration as to physical fitness & Medical Certificate from Chief Medical Officer in-charge of a referral hospital or Sub Divisional Hospital or Rural Hospital or Civil Hospital or Civil Hospital).
9. After verifying the medical certificate, if the Licensing authority has reason to believe that the applicant is physically unfit to perform the duties of a conductor, it may call upon him to furnish a third copy of his clear and recent photograph in addition to the photographs already furnished and to produce another medical certificate of fitness from a Medical Officer of the District concerned for this purpose and the photograph so furnished should be firmly affixed with the application duly signed and sealed by the registered medical practitioner.
10. If the licensing authority is satisfied with your application and the medical fitness, then applicant will be provided with conductor license.

Required Documents

1. Application Form : Form
   
2. Two clear copies of recent photographs of the applicant.
   
3. Medical certificate
   
4. Affidavit of address proof
   
5. Proof of age, appropriate for relevant class of vehicle.
   
   • Ration card, Voter identity card, Life insurance policy, Passport, School certificate, Birth certificate or
     
   • Certificate granted by a registered medical practitioner not below the rank of a Civil Surgeon(as applicable) or
     
   • Self-swearing Affidavit by an applicant before an executive Magistrate or a First Class Judicial Magistrate or a Notary Public as evidence of age
     
6. Proof of residence
   
   • Ration Card, Voter identity card, Aadhar Identity Card, Pay slip issued by any office of the central government or a state government; Electricity bill, Telephone bill, House tax receipt showing the name of the name of the applicant
     
   • If the applicant is not able to produce any of the above documents for a sufficient reason the licensing authority may accept any affidavit sworn by the applicant before an Executive Magistrate, or a First Class Judicial Magistrate or a Notary Public as evidence of residence
     
7. Aadhaar Card
   
8. A cash receipt or a treasury challan in token of payment of fee.

Eligibility

1. Candidate must be matriculate
   
2. Candidate must possess knowledge of the provisions of the Act and the rules made thereunder
   
3. Candidate must be conversant with the duties and functions of a conductor
   
4. Candidate should have a sound physique and
   
5. Must have satisfactory proof of his good character.

Validity

• Conductor’s License is valid for a period of three years from the date of issue and eligible to use throughout the State.

Need for the Document

1. No person shall act as a conductor of a stage carriage unless he holds an effective conductor’s licence issued to him authorising him to act as such conductor.
   
2. No person shall employ or permit any person who is not so licensed to act as a conductor of a stage carriage.

• A conductor’s licence may at any time be revoked by any licensing authority if that authority has reasonable grounds to believe that the holder of the licence is suffering from any disease or disability
