Procedure

1. Skopje appointment by telephone toll free number 0800 192 92 is mandatory for all applicants.
2. After the appointment, approach the Department of Vehicle and Driver license where you have your appointment. You could also use the contact information provided under heading Office Locations & Contacts in this page and get more information.
3. Also, make sure you have all the documents listed under Required Documents section of this page except the application form before approaching.

Required Documents

• Valid identity card for inspection.
• Announcement of the lost license in Fig. Vesniik of RM
• A written statement of the driver with a brief description of the time and place of loss of driver's license.
• If a driver is lost past importance and that it includes a certified "D" category the person should submit a medical certificate.
• Prijava for driving booklet
• Uplatnica 100, oo day
• PHOTOCOPY of ID
• PHOTOCOPY of license
• 2 photos to dimension 3,5 X 4,5 CMM

Eligibility

• Duplicate license are issued when the original license is lost or stolen.

Fees

• Application fee 280, oo day.
• If a lost driver's license is valid payment of 150, OO Day
• If the license is valid past it requires payment of 210, oo denars.

Validity

Explain the time until which the certificate/document is valid.
