Lithuania - Register a Death
ProcedureEdit
In general, a death must be reported within five days to the registrar of births and deaths at the register office where the death took place. It is also possible to register the death at a register office in a different area, but this may slow down the funeral arrangements. In some areas, it may be necessary to make an appointment beforehand.
The person registering the death should take the medical certificate signed by a doctor and if available, the NHS medical card, birth certificate and marriage or civil partnership certificate of the deceased.
In the event of a death of a family member, friend or relative , the first thing to do is to call the family doctor. The doctor will provide a medical certificate stating the cause of death and a formal notice confirming that they have signed the medical certificate. This notice also explains how to register the death. If the death has occurred in suspicious circumstances, or it is that of a stranger, it should be reported to the police. If the cause of death is not clear, it must be reported to the coroner who will decide if a post-mortem and inquest are required.

EligibilityEdit
Death shall be registered in the offices or one of the offices referred to in Article 3.280 of the residence of the deceased on the basis of the medical certificate of death.
Occurrence of death shall be notified and registered within three days of the death or the time when the dead body was discovered.

While registering death, the register office shall make a death record and issue a death certificate.
In the event of a stillbirth the registration must take place within 42 days or three months at the latest, either at the hospital or register office.
Required InformationEdit
- Name of the deceased. Surname and Given Name(s).
- Father's Name.
- Mother's Name.
- Spouse Given Name(s) and surname of surviving spouse, if listed.
- Date of Death Day, month and year of death, in dd/mm/ccyy format.
- Cause of Death.
- Age of the deceased.
- Place of death Town, Uyezd (district) and Gubernia (province).
- Place of residence The town where the deceased resided.
Need for the DocumentEdit
Death shall be registered upon the application of the relatives or neighbours of the deceased or of the owner of the home where the deceased person lived, as well as on the notification by the administration of the medical centre where the person died or the police commissariat.

In most cases, the death is registered by a relative. If there are no relatives and the death occurred in a house or hospital, the following people may register the death:
- official from the hospital
- the person organising the funeral or the funeral directors
- a person present at the time
- someone living in the house
