Kosovo - Correct and Change Last Name on Identity Card

ProcedureEdit

  1. Applications to correct and change last name on identity card must be personally submitted by the citizen to the department in your local municipality that processes ID card application. For minor applicants, the presence of both parents at the time of application is required.
  2. Complete and submit an application form together with all other required documents:
    1. Birth certificate,
    2. Identity card issued by UNMIK,
    3. Valid identification: Identity card, military card, passport, driving license, index or another document of foreign county,
    4. Recent utility bill with address: electricity, phone, water, waste, tax bill or house or apartment contract
    5. UNMIK ID of the person on whose name the bill is.

Correction and change of last name in identity card:

  • Birth certificate,
  • Identity card issued by UNMIK,
  • Valid identification: Identity card, military card, passport, driving license, index or another document of foreign county,

Note: The same proofs serve for the persons born after year 1970, who previously didnt have identity documents.

Need for the DocumentEdit

An identity card is a document issued to a citizen stating the holders' name, citizenship, gender, date of birth, place of birth and the address where the holder resides.

Identity cards can be presented as identification document when you cross state borders, as long as there is an existing agreement between states.

In cases, where the holder changes his/her name and surname; or changes permanent residence; or the picture on the ID no longer shows the holders true appearance, a new ID card must be issued. A citizen is given 30 days to report the change and to request for a new ID card.
