Apply In-Person

  1. Registration of Death and certificate issuance will be processed by respective municipal office.
  2. A death can be reported and registered
    1. by the head of the family, in case it occurs in a house.
    2. by the medical in-charge if it occurs in a hospital.
    3. by the jail in-charge if it occurs in a jail.
    4. by the headman of the village or the in-charge of the local police station in case the body is found deserted in that area.
  1. Please visit the respective municipal office. Registration of death shall be applied by hospitals, medical or other institutions or by an individual.

  4. Submit the required documents and pay required fees if required.
  5. Authorities will verify the details and applicant will get death certificate by evening.
  6. Alternatively you may check record online and get print out if it is sufficient to serve your purpose.

  1. Death certificate can be downloaded from “Sevana website” of Kerala Government Civil Services online portal.

  3. Click on “Certificate Search” option from the list of options provided to reach the next page.
  4. Select your district, Local body type and Local body from the drop down menu. Click on submit for list of certificate as per records year wise.
  5. Select year of death form the list to get page with prompt for user to enter details about the deceased person and hit search to get he certificate printed.
  6. Please use the appropriate option listed to get print out if it is sufficient to serve your purpose.



Apply online:

  1. Visit the following portal: portal.
  2. Click on “citizen login” and fill in the required details to sign up.
  3. Upon successful registration, an acknowledgement will be sent to the mobile number.
  4. Login again on the portal. Click on “services” and choose the desired service.
  5. Fill the required details as asked and click on the “submit” button.
  6. Upload the required documents and the form will be submitted successfully.
  7. Follow the notification for further instructions from the concerned authorities.



Apply through Centres:

  1. To apply through centres, the applicant has to visit the respective “CSC” centre. Please tap on the following link to locate the centre according to the respective district: link.
  2. Make sure to have the documents that are listed under the “Required Documents” section of this page.
  3. It is advised to take the documents in original to the kiosk for scanning purposes.
  4. The operator will process the application online.
  5. On successful submission, the applicant will receive an application number as acknowledgement.
  6. The status, viz. Approval or Rejection if any, will be conveyed to the applicant through an SMS.
  7. If verified, the registration certificate will be hand delivered or sent through post to the address mentioned while applying in a stipulated period of time.

EligibilityEdit

  • Death certificate will be issued to any citizen or any foreigner's legal heir, the death of whose relative takes place in the State.
  • Parents and relatives of the deceased are eligible.



FeesEdit

  • No fees for those registered within 21 days of such occurrence.
  • On delayed registration after 21 days of occurrence, applicant has to pay as advised for a nominal charges.
