Kerala - Change Family Head Name in Ration Card

ProcedureEdit

Apply In-Person:

  1. In order to change the family head name (change of ration card holder) in the ration card, the applicant shall approach their respective ration shop or respective local TSO(Taluk Supply Office)/DSO office in their respective area as applicable.

  3. Go to the respective office and collect the application form or write on a plain A4 sheet as advised.
  4. Complete the application form. Attach the required documents as per our “Required Documents” section of this page and submit it to the designated receiving authority.
  5. Authorities after checking the submitted application along with attached documents will make entries and provide you record number for reference.
  6. Keep it safe for future reference.
  7. This application will be processed further to issue the ration card with updated family head name as per notified period.
  8. The ration card will be delivered as per applicable norms.


Apply through Akshaya

  1. Applicant must visit the nearest Akshaya center. Link for center link
  2. Applicant shall obtain application form from the centre.
  3. Submit the completed application, document and applicable fees to the centre staff.
  4. Application submitted will be processed and receipt with application number will be issued as acknowledgement. Please keep it safe for future reference.
  5. Application will be processed by the respective department. “SMS” triggered update on application status will be sent to applicant mobile.
  6. Please follow the notification to get the updated ration card as per deliverable norms.



Apply Online

  1. To Change Family Head Name in Ration Card, the applicant has to make the application online via e-Services - Ration Card Management System link
  2. The applicant has to login via user id and password and enter captcha and click “Sign In”. if the applicant doesn’t have an account, then the applicant can click the “create an account” to create a new one.
  3. After logging in the applicant has to select the service and enter their ration card number for further process (If applicable)
  4. Then proceed by clicking on the option Apply for change name on Ration Card.
  5. Then applicant has to upload the required documents that are mentioned in the “Required Documents” section of this page (If required).
  6. Check all the information you have entered before submitting.
  7. Click the submit button and take a printout of the application form for future use.
  8. Note down the application number and date.
  9. The applicant is informed through SMS at various stages i.e., Advance intimation about date of visit of the official to his place of residence for verification of his application, Approval/Rejection of his application.
  10. After ground level verification ration card will be issued in 2 – 3 weeks as notified by the department



Apply through Centres:

  1. To change family head name in ration card, the applicant has to approach nearby Common Service Center (CSC) Link. Also, the applicant can approach nearby Akshaya service centre link
    • Note: The applicant has to select their state, district and block from the drop-down menu or the applicant can click on their respective district to find nearby center.
  2. An application form will be provided to the applicant by the concerned officer. Or the applicant has to provide their details to the concerned officer.
  3. The applicant has to fill in all the necessary details in the application form.
  4. The applicant has to attach or provide the required documents that are mentioned in the “Required Documents” section of this page (If required).
  5. Submit the completed application, document and applicable fees to the centre staff.
  6. Application submitted will be processed and receipt with application number will be issued.

  • For implementation of the Targeted Public Distribution System(TPDS), Ration Card is being issued to all the heads of families, with details about family members included in the card. In some states, the government grants financial sanctions and other benefits to the woman family head as per ration card. To avail those benefits, the family head in the ration card need to be changed from respective husband to wife.
