Online Process
- For online processing of death certificate registration and to obtain printout, please use the following link: Online Portal
- One has to login to the page using the registered user name and password or else please register and login
- Once logged in, please select ”Death registration” option displayed to reach the next page.
- In the given page, please enter / select appropriate details and upload required documents to register.
- Once you hit submit button you will get a acknowledgement and please keep it safe. The same will be sent to your mobile number that was provided while applying for future reference
Apply In-Person
- To apply for a Death Certificate, you must first register the Death.
- The death has to be registered with the concerned local authorities within 21 days of its occurrence, by filling up the form prescribed by the registering officer.
- As a first step a death should be notified to the respective authorities at the earliest it occurs.
- A death can be reported and registered
- by the head of the family, in case it occurs in a house.
- by the medical in-charge if it occurs in a hospital.
- by the jail in-charge if it occurs in a jail.
- by the headman of the village or the in-charge of the local police station in case the body is found deserted in that area.
- Once the information is registered / reaches the Municipality or MRO, the concerned citizen has to approach the authority and apply for the certificate on any working day.
- Applicant shall use the respective form as per the link to apply, If he wish to register on his own: Form2, Form 4 and Form 4A: Form 2 and Form 4 Form 4A
- Once the completed form along with documents were submitted, Authorities will accept the form verify the details to process and issue the certificate.
- OR
- If you report orally, the registrar will enter the information in the appropriate reporting form and get your signature or thumb impression. You may have to furnish receipt from crematorium or burial ground to get the certificate.
If the details are registered already:
- Please go to the District Statistical Office and apply to check for the records.

- If the search results in availability of Death details, authorities will prepare Death certificate after checking the documents submitted.
- Note: In case the death has not been registered within the specified time of its occurrence, the certificate is issued after due police verification ordered by the revenue authorities.

- One has to login to the page using the registered user name and password
- Once logged in, please select ”Death Certificate” option displayed to reach the next page.
- In the given page, please enter / select appropriate details and hit Search to get the next page
- A registration number will be returned for the details provided. Please click the registration number to get the certificate.
- Please hit “Print” button to get the printout of the certificate

- Any citizen or any foreigner's legal heir, the death of whose relative takes place in the State.
- Parents and relatives of the deceased.
FeesEdit
- No Fees required if registered within 21 days.
- There will be a small amount collected for people registering after 21 days
- There will be a fee for additional copies of printout

- The persons are required to register deaths within the prescribed period of 21 (twenty one) days from the date of death as the case may be.

- The Chief Registrar of Births and Deaths and the Director of Economics and Statistics.
In Rural areas:

In Urban areas:

- In case of City corporations: Health officer.
- In case of City Municipal councils: Health officer/Health inspector.
- In case of Town Municipal councils: Health inspector.
- In case of Notified areas/ Project areas/Cantonment - Health inspector.

Need for the DocumentEdit
- As per the Registration of Births and Deaths Act 1969, it is mandatory to register every death with the concerned State Government within 21 days of its occurrence. If not registered within 21 days, permission has to be obtained from the Registrar/Area Magistrate by paying prescribed fee for late registration and can get death certificate.
- A Death Certificate is a document issued by the Government to the nearest relatives of the deceased, stating the date, fact and cause of death.
- It is essential to register death to prove the time and date of death, to establish the fact of death for relieving the individual from social, legal and official obligations
- To enable settlement of property inheritance, the family to collect insurance and other benefits.
- Death certificate is an important official document issued for the purpose of settlement of all the belongings and benefits of the deceased individual to the relatives
