Online Process
- For online processing of birth certificate registration and to obtain printout, please use the following link: Online portal
- One has to login to the page using the registered user name and password or else please register and login
- Once logged in, please select ”Birth registration” option displayed to reach the next page.
- In the given page, please enter / select appropriate details and upload required documents to register.
- Once you hit submit button you will get a acknowledgement and please keep it safe. The same will be sent to your mobile number that was provided while applying for future reference
Apply In-Person
- To apply for a Birth Certificate, you must first register the birth.
- The birth has to be registered with the concerned local authorities within 21 days of its occurrence, by filling up the form prescribed by the registering authority.
- Normally the dispensaries or hospitals where the birth takes place maintain a register and the same is sent to the local bodies at regular intervals for registering the records.
- Once it reaches the Municipality or MRO, the concerned citizen has to approach the authority and apply for the certificate on any working day.
- Applicant shall use the form as per the link to apply on his own: Form link
- Once the form along with documents were submitted, Authorities will accept the form register the birth by verifying the details and documents to issue the certificate.
If the details are registered already:
- Please go to the District Statistical Office and apply to check for the records.

- If the search results in availability of birth details, authorities will prepare birth certificate after checking the documents submitted.
- Note: In case the birth has not been registered within the specified time of its occurrence, the certificate will be issued after due police verification ordered by the revenue authorities.
Print Certificate
- For online printout, please use the following link: Portal ink
- One has to login to the page using the registered user name and password
- Once logged in, please select ”Birth Certificate” option displayed to reach the next page.
- In the given page, please enter / select appropriate details and hit Search to get the next page
- A registration number will be returned for the details provided. Please click the registration number to get the certificate.
- Please hit “Print” button to get the printout of the certificate
Apply online
- To Obtain a Birth Certificate, the applicant has to make the application online via link – link
- If the applicant is a new user click the “Register HERE” button.
- The applicant has to enter the Aadhar number in the newly opened page.
- The applicant will get the OTP from the registered mobile number.
- The applicant has to enter the OTP number and click the “Allow” button.
- The applicant has to enter all the Email ID, Password, Phone number and click the “Submit” button.
- Now, the applicant has to login and click the “LOGIN IN”.
- The applicant has to select the respective department and the applying certificate.
- Enter all the details in the application form.
- Make sure that the applicant has to upload all the necessary documents which is mentioned in the “Required Documents” section of this page.
- The applicant has to pay the prescribed fee as requested (If Applicable).
- After completion, the applicant has to click the “Submit” button.
- The applicant will receive an application number as acknowledgement. The applicant will also get a transaction number as a text message on their registered mobile number.
- Once the verification process gets completed by the respective officer.
- ”SMS” triggered update on application status will be sent to the applicant’s mobile. Please keep track of the notification.
- The submitted application will be processed by the department. A designated authority (supply officer) will verify the submitted documents and inspect the area of the applicant to clarify the applicant’s details.
- Based on the outcome of the above-mentioned steps and other internal processing, the designated issuing authority will decide on the issuance of the Certificate.
- The status, viz. Approval or Rejection will be conveyed to the applicant through an “SMS” or applicable norms.

Apply Through Nadakacheri Centres
- To Obtain a Birth Certificate, the applicant has to approach respective “Nadakacheri” centre link
- Please visit the centre and consult with the centre operator for your requirement.
- If the operator issues an application form, then collect it and pay the requisite fee. Please complete the application form.
- Now, submit the completed application along with the required documents as per the “Required Documents” section of this page. It is advised to take the documents in original to the kiosk for scanning purposes.
- The operator will process the application online. On successful submission, the applicant will receive an application number as acknowledgement. The applicant will also get a transaction number as a text message on their registered mobile number.
- Applicant has to make necessary payment as centre operator quotes and don’t forget to take the receipt.
- This application will be forwarded to the respective department for further processing.
- ”SMS” triggered update on application status will be sent to the applicant’s mobile. Please keep track of the notification.
- The submitted application will be processed by the department. A designated authority (supply officer) will verify the submitted documents and inspect the area of the applicant to clarify the applicant’s details.
- Based on the outcome of the above-mentioned steps and other internal processing, the designated issuing authority will decide on the issuance of the new Death Certificate.
- The status, viz. Approval or Rejection will be conveyed to the applicant through an “SMS” or applicable norms.
Required DocumentsEdit
- If the birth takes place at a dispensary/hospital, the register record is enough.
- Address proof(Voter ID, Driving License etc.) of parents
- Aadhaar Card of Parents
- If the birth takes place at a house, the citizen has to approach the local body and inform in writing. Then it will be verified & the certificate will be issued.
- Application is to be made in the prescribed format of Birth Report Form-1. Link for form : FORM 1

- Certificate of Doctor / Hospital discharge certificate
- In case of delayed registration, an affidavit from judicial magistrate / Notary is required indicating the date and place of birth, names of parent and evidence of the event of birth
- In case you have applied after a month but within a year you have to obtain a permission from the Add.Dist.Registrar i.e BDO of concerned Taluka
- In case applied after one year, need to obtain a order from executive magistrate and copy of the said order is required to be attached with the prescribed application form.

- In India, it is mandatory under the law (as per the Registration of Births & Deaths Act, 1969) to register every birth with the concerned State/UT Government within 21 days of its occurrence.
- The Government accordingly has provided for a well-defined system for registration of Birth, with the Registrar General at the centre and the Chief Registrars in the States, running through district registrars to the village and town registrars at the periphery.
Issuance of birth certificate in the following locations:

- In government hospitals
- Bangalore Mahanagar Palike larger the designated clinic
- The Corporation's main office
The birth certificate is a very important document which serves various purposes throughout life. This is the main document that proves your age. Birth registration is compulsory and when the birth was registered within 21 days, so it is a very easy process. Between 21 to 30 days after birth and shall certify MOH will charge a penalty of Rs 25. After 30 days and within a year the Joint Director, Statistics can issue a birth certificate. 50 rupees for it and fined an affidavit (affidavit) is required to submit.
