Apply Online
- Go to link and click on Departments & Services
- Click on Planning Programme Monitoring & Statistics Department and select Application for Death Certificate. Alternatively, you can search for Application for Death Certificate in the search option.
- Click on "Apply online" button.
- Enter the username, password/OTP, and captcha and click on Log In button
- If you have the Registration No. of the previously issued certificate, select “Yes”. Enter the Registration Number and click on Search to get the details.
- If don’t have the Registration No. of the previously issued certificate, select “No”. Enter the Date of Death, Name of Deceased, Father’s Name and Mother’s Name and click on Search to get the details.
- Verify the details. If the details are correct, select the checkbox (“Yes”) and enter the No. of copies for Print. Enter the captcha and click on Submit to proceed further.
- Verify all the details and click on Make Payment to proceed. Select the Mode of Payment and click on Make Payment to proceed
- After payment is successful, an acknowledgment will be generated. Acknowledgment consists of applicant details, application details and payment details for the applicant’s reference.
- To download the certificate, go to the link
- Click on Track your application status. Select the option as appropriate and click on "Submit" button.
- Check Current Status of the application. If it is delivered, Click on Delivered.
- Under Issue Document(s), click on Output certificate
- Death certificate will be downloaded. Click on print option to print the certificate
Apply through CSC
- To Download or View Death Certificate, the applicant has to approach the nearest CSC link
- NOTE: To check the contact details of CSC centre, please select the District and Block.
- The applicant has to visit the nearest centre.
- The applicant has to inform all the details to the respective officer.
- The officer will enter the details in the portal with the given details.
- The applicant will get the downloaded certificate from the officer.
- To Download or View the Death Certificate, the fee is free of cost.
- The processing time for this procedure is within a minute.

- A death certificate is either a legal document issued by a medical practitioner which states when a person died, or a document issued by a government civil registration office, that declares the date, location and cause of a person's death, as entered in an official register of deaths.

- Death certificate helps in closing bank accounts, claiming insurance, selling property of the deceased or any work related to banks. It is also required for property transfer or property tax payment. If the deceased is the owner of a property, then succession certificate will be required in the absence of a 'Will'.
