Procedure

Apply In-Person:

1. To Change Date of Birth in Birth Certificate the applicant has to approach the hospital where the birth takes place and collect proof of correct date.
2. An application form will be provided to the applicant by the concerned officer.
3. Please complete the application form and attach the required documents and submit it to the receiving authority
4. The accepted application and documents will be verified by the authority and a acknowledgement will be provided for applying.
5. Collect the acknowledgment slip after submitting the form and keep it safe for future reference.
6. This application will be processed further by the respective department.
7. Once the process is over, applicant gets notification to collect the certificate.
8. Use the acknowledgment slip to obtain certificate from the office as per notified period.
   • Note: These changes will be published in a local newspaper and you’re with the respective state official Gazette publication.

• Affidavit from local notary
• Address proof (Voter ID, Driving Licence etc.) of parents as per requirement
• Aadhaar card
• Education Certificates as required
• Aadhaar Card of Parents as per requirement
• Report from Medical Institution who conducted delivery. (As per requirement)
• Affidavit from two credible persons. (As per requirement)
