• Contribution suspension statement.
• Photocopy of the insured ID card for Jordanians and the passport for the non Jordanians.
• The insured/his agent are banking account number.
• End of service letter or resignation.
• A letter from civil or military retirement administration or retirement decision for the insured.
• Medical reports, which declares the insured health condition.

Eligibility

• Ending his/her service due to health reasons.
• Applying for disability pension before reaching the statutory age (60 for males and 55 for females).
• Contribution period should be no less than 60 installments, 36 of them periodical.
• Proving the disability from a medical reference.
• The insured has no right to claim for disability pension for the previously mentioned disability cases due before coverage by social security law.
