• Deduction discontinuation statement issued by the establishment.
• Decease certificate.
• Succession limitation deed.
• Custody deed (if there are any underage survivors).
• The deceased valid family book and the parents valid family book(if available), or the family card, passports and birth certificates for the non-Jordanian.
• Accounts' numbers for the entitled survivors or their attorney.
• Judicial power of attorney.
• The ID Card of the Jordanian attorney or the passport of the non-Jordanian.
• A letter issued by the Directorate of Civil and Military Retirement or the retirement decision for the Jordanian.
• A deed of support issued by the competent court (only if any brothers or sisters available).
• The family book of the Jordanian brothers and sisters, and the passports for the non-Jordanians (only if any brothers or sisters available).
• The judicial medical report indicating the death cause and police investigations on the incident.
