Procedure

Apply In-Person:

1. To apply for a health certificate for Animals, the Applicant must visit the Veterinary Hospital of their respective area. The address and contact details of all the Veterinary Hospitals of Jharkhand can be found in this Contact Link
2. Collect the applicable application form from the Veterinary office.
3. Fill up the application form with the correct information.
4. Attach the required documents as per the “Required Documents” section below.
5. Submit the application form to the office and make the required payment.
6. After submission, the Applicant has to wait for 2-3 days.

Apply Online:

1. To Apply for Health Certificate for Animals in Jharkhand, the applicant shall visit the following webpage: Link.
2. A new page will open, where the applicant must log in using their login credentials.
3. After login, the applicant shall choose an appropriate option to obtain the certificate and proceed further.
4. The applicant shall fill up the application form with all the necessary details on a newly opened page and complete the given application form.
5. Now the applicant shall upload scanned copies of all the "Required Documents" for further processing.
6. Once the documents are uploaded, provide the mobile number and enter the received OTP.
7. Now click on "Submit" and complete the process.
8. On successful verification, the certificate will be issued by the authority.
9. Obtain the certificate from the concerned office or the portal.
   
   

Apply through Centres:

1. To Apply for Health Certificate for Animals in Jharkhand, the applicant shall visit the nearest CSC/Tehsil Centre. Find contacts to CSC centres in the link provided:Link
2. Procure the application form from the concerned centre and fill it up with the necessary details required for the card.
3. Make sure to attach all the "Required Documents" along with the application form before submission.
4. Provide authorized signatory and photograph, wherever applicable.
5. Now submit the application to the office of the concerned centre.
6. On successful verification, the applicant will be notified by the CSC centre.

• Xerox copy of license register for animal
• Laboratory Reports (not mandatory in some cases)

• Residential proof of the owner

  • Telephone (landline or post-paid mobile bill)

  • Income Tax Assessment Order

  • Certificate from Employer of reputed companies on letterhead

Eligibility

Given below are the eligibility criteria to avail of the benefits of this certificate.

• Animal health certificate only applies to those animals whose country of origin is India.
• Must be a licensed owner of an Animal with Pin number
• Must have a Vaccination card (against any diseases) of that particular animal.

Fees

• Payment will be made as per the requirement of the Office.
• Additional fees may be charged for the process.

Validity

• The Certificate is valid for Four months
