Jamaica - Reregister a Birth

Apply In-Person
- To apply for re-registration of birth , you or your declarant should visit the Register General Department (RGD) office, please view the RGD contact details and location map via this ink: Link and the RGD Branch contacts via this link: Link Note that a declarant can be a Mother or father or legal guardian of the child or a person with presenting a power of attorney or a person by order of court of law.
- Obtain the application form for re-registration of birth form the RGD office or download it directly via this link: Link
- Read the form keenly and fill it correctly as instructed in the presence of a Justice of the peace (JP) or Notary public. Note that the the dates signed by the Notary Public/J.P. must be the same as the date signed by you or the Declarant and if errors were committed while completing the form it must be crossed out and initialed by the same signing Notary Public or J.P.
- Then Prepare all the required documents and attach them to the application form
- Visit the RGD office in your region and submit the complete application to the RGD official at the service desk for verification.
- Once the RGD officer is satisfied the application is to the required standard, you will be directed to the RGD cahier window to pay for the application according to the service you wish to apply for i.e. normal or express service.
- Upon payment, you will be issued with an acknowledgment receipt confirming your application and the RGD will issue an authority for child’s birth to be re-registered at the Local District Registrar (LDR) or Registration Centre where the birth occurred.
- The LDR will process the new birth certificate which you will be required to collect at their office by presenting the necessary document issued by the RGD at the time of your application.
Application by a Proxy
- If you are not able to apply by yourself, you can choose a third party to submit the application on your behalf. However, the third party must present a letter of authorization with these details; name, address and phone number of the owner of the account (i.e. the child, parent, husband or wife) and the must also show approval for the party to conduct the transaction on your behalf. Note that the letter must be certified by a Notary Public or a Justice of the Peace J.P

- Valid picture identification for the applicant, mother and father

InstructionsEdit
- All signatures should be the same as on the ID
- ID for the Declarant and Applicant must be provided for processing an application.
- All errors must be crossed out and initialled by the same signing JP/Notary Public
- All information should be clearly written.
- All photocopied IDs and supporting documents, except those issued by the Registrar General’s Department must be certified by the J.P./Notary Public.
- If a declarant with an overseas address signs before a J.P in Jamaica, the words: “Whilst on visit/trip to Jamaica” must be inserted beside the signature of the declarant
- Incomplete applications will not be processed
