How to Apply

  1. For correcting errors on birth record , you or your declarant should visit the Register General Department (RDG) office, please view the RDG contact details and location map via this ink: Link and the RDG Branch contact via this link: Link Note that a declarant can be a Mother or father or legal guardian of the child whose record will be amended or Credible person with a power of attorney (Preferable Relatives)
  2. Obtain the application form for correcting of error on birth record form the RDG office or download it directly via this link:Link
  3. Read the form keenly and fill it correctly as instructed in the presence of a Justice of the peace (JP) or Notary public. Note that the the dates signed by the Notary Public/J.P. must be the same as the date signed by the Declarant or you and if errors were committed while completing the form it must be crossed out and initialed by the same signing Notary Public or J.P.
  4. Then Prepare all the required documents and attach them to the application form
  5. Visit the RDG office in your location, pay the application fee according to the period of service you wish to apply for i.e. normal or express service
  6. Submit the complete application to the RDG official at the service desk for verification and processing. Note that a declarant will be subject to an interview with the RDG before processing of the application. Interview sessions for the signing Declarant are Mondays to Fridays, 8: 30a.m 2:00p.m.
  7. Upon a successful application, the RGD will process your request and amend the birth record.


Application By a Proxy

  • If you are not able to apply by yourself, you can choose a third party to submit the application on your behalf. However, the third party must present a letter of authorization with these details; name, address and phone number of the owner of the account (i.e. the child, parent, husband or wife) and the must also show approval for the party to conduct the transaction on your behalf. Note that the letter must be certified by a Notary Public or a Justice of the Peace J.P.

  • Valid ID (Passport, Driver’s License/Voters ID)
  • T.R.N. and N.I.S.
  • First Primary school record

  • All recorded births in Jamaica are eligible for correction in an error was committed while entering the datils on a birth record


FeesEdit

  • Normal service: $4,500 (4-6 weeks)
  • Express service: $6,500 (7-10 working days)
  • For any Additional copies cost $500 each at the time of application

  • Four to six weeks for the normal service and seven to ten days for the express service, however the interview process for declarants might affect these dates

InstructionsEdit

  • All signatures should be the same as on the ID
  • ID for the Declarant and Applicant must be provided for processing an application.
  • All errors must be crossed out and initialled by the same signing JP/Notary Public
  • All information should be clearly written.
  • All photocopied IDs and supporting documents, except those issued by the Registrar General’s Department must be certified by the J.P./Notary Public.
  • If a declarant with an overseas address signs before a J.P in Jamaica, the words: “Whilst on visit/trip to Jamaica” must be inserted beside the signature of the declarant
  • Incomplete applications will not be processed
