Procedure

Apply In-Person

1. To apply for substituting a driver’s license that you have misplaced, lost or stolen, you should visit the Island Traffic Authority (ITA) to submit your application. You can view the ITA office contacts via this link: Link
2. But first report the circumstance to which licence is lost, misplace or stolen to the nearest police station
3. Then the police will issue a letter which you will need to submit to the ITA office along with two identical passport photographs
4. Then the ITA will check into their data system any judicial endorsement (if your Driver’s license is suspended by the judicial)
5. If not found, the ITA will give you a go ahead and complete your application with the Tax Administration Jamaica (TAJ)
6. Then obtain the substitute application form from the ITA office or download it form the TAJ website: Link (scroll down to find “Application for Substitute Driver's Licence.”) and fill accordingly
7. Then take the complete application to the tax office where the first licence was issued or where you did you last addition or upgrading on the license
8. The TAJ official will verify your application and confirm with their database for competence and particulars regarding your driver’s license
9. If the information is found and certified, you will be issued with the relevant form and maybe ask to take a Reading test and then you will be directed to pay the application fee
10. Once you have paid the application fee, the TAJ official will take your photograph and the process your application and you will be issued a new driver’s license

Note that:

1. If your information is not found in the TAJ data base, you will directed to return to the Tax office after five days in order to allow the TAJ officials to search for your information manually and if still the information is not found after the manual search you may be direct to the ITA examination deport where the license was issued or where you last upgraded the license for processing and the license will be issued to you after the relevant information is found and you have paid the application fee
2. Where a Judicial endorsement is found, the ITA will send a letter to the Tax office (TAJ) to regarding the endorsement. And you will then be informed of the duration and the circumstances surrounding the suspension of your application and the demerits that you have earned and then the letter is placed on the suspension file and update on the electronic system.
3. At the end of the suspension, visit the court to obtain a letter form the court stating that your suspension is over and that you are now eligible to receive a driver’s license and take the letter to the TAJ office for verification and recording and then the license will be issued to you once you have paid the application fee
