Procedure

Apply In-Person

1. To apply for a provisional driver’s license, you should visit the Island Traffic Authority (ITA) to submit your application. You can view the ITA office contacts via this link: Link
2. Obtain the provisional driver’s license application form at any office of the Island Traffic Authority (ITA), Tax Office or download it from the website of the Ministry of Transport: Link (Form H3A - Application for Provisional Drivers Licence) and Mining or Tax Administration Jamaica (TAJ): Link (Provisional Driver's Licence). 3. Fill the application form accordingly and then have it certified by a Justice of the Peace (JP) official or a Superintendent of the police.
4. Obtain three identical photographs and have certified by a JP or a police superintendent.
5. Contact the ITA office to book an appoint for a road code test.
6. On the appointment date, visit the ITA office, examination deport and submit your complete application form alongside the certified photographs to the ITA official at the service desk
7. The official will verify your application and direct you to the examination room where your reading ability will be tested.
8. Then you will take the Road Code Test, which is designed to evaluate your knowledge of the general rules of the road which would include road signs, road markings and road safety.
9. After taking and passing the test, the ITA will record your results in their system and forward your application to a specified Tax Administration Jamaica (TAJ) office for processing of the provisional driver’s license and you will be issued with some documents, to be presented to the tax office to be validated.
10. Visit the specified tax office in your location and pay the provisional licence fee at the cashier and obtain the payment receipt
11. Then head to the TAJ section in charge of licenses and submit the document file issued by the ITA and the payment receipt to be verified by the agent responsible.
12. The TAJ agent will verify your documents and confirm with the ITA data base and then issue you a provisional driver’s license.

Instructions

• You are required to pass the Road Code Test in order to be issued the provisional driver’s license and the passing mark is 75%
• Should you fail to pass the test you can reapply for a new test
