Procedure

Apply In-Person

1. To Apply for the National Health Fund (NHF) Card, you should submit your complete application to the NHF office or at any Card Centre in the parish you are residing in. Please view the NHF contact details and Card Centres in the “Office Location and Contacts” section below
2. Obtain the application form for the NHF card at any of the NHF offices or at any card centre or download the form from the NHF website: [www.nhf.org.jm Link] or download the form directly via this link: Link/images/forms/New_NHF_Application_Form.pdf Link]
3. Fill the application form correctly and then take it to your doctor to certify the application by fill the required section for doctors
4. The submit the complete application to the NHF office near you are at any Card Centre
5. Upon submission of the application, the NHF officials will verify the information you have provided to check if the form is completed accordingly
6. Once verified, your application will be approved and you will be registered for NHF benefits then your NHF Card will be proceed and issued to you

Eligibility

The NHF Card Programme is without age restrictions and provides subsidies to beneficiaries for the treatment of 17 chronic illnesses. These include:

• Asthma & Subsidy for Spacers & Masks
• Arthritis
• Breast cancer & Breast Cancer Receptor Studies Test
• Benign prostatic hyperplasia
• Diabetes & Diabetes Programme
• Epilepsy
• Glaucoma
• High Cholesterol
• Hypertension
• Ischaemic heart disease
• Lupus
• Major depression
• Prostate cancer
• Psychosis
• Rheumatic fever/heart disease
• Sickle cell disease
• Vascular disease

Instructions

• Please keep your card safe and should get lost or damaged report to the NHF office immediately for replacement of a new card
