Apply In-Person

  1. To register for National Insurance Scheme (NIS) as an Individual, you should visit the NIS office under the Ministry of Labour and Social Security (MLSS). Please view the NIS contact details in the “Office Locations &Contacts”section below and the MLSS contact details via this link:Link
  2. Obtain the NIS Individual Registration form from any of the NIS offices or download the form from the MLSS website through this link:Link (Select “NIS Individual Registration” to download the form)
  3. Read and complete the registration form as instructed.
  4. Attach the required supporting documents to the application form as mentioned in the “Required Documents” section below
  5. Go to the NIS office in your parish and submit the complete application to the NIS official at the service desk
  6. The NIS official will verify your application if it meets the required standard and enter your details into their system for processing
  7. Then the application will be processed and you will be registered with the NIS
  8. After the registration process, you will be issued with your unique NIS registration number
NOTE:
  • Married women must also submit the original or notarized copy of their marriage certificate if their proof of name is the birth certificate or if their married name is not recorded in their passport.
  • If you are unable to apply by yourself, you can send a third party with an authorization letter to act on your behalf.

All persons who are gainfully occupied in insurable employment are eligible to apply with the age limit of 18 to 70 years for males and 18 to 65 yrears for females.
