Jamaica - Apply for Life Certificate under National Insurance Scheme

Apply In-Person

  1. To apply for the Life Certificate under the NIS, you have to submit your application to the NIS office under the Ministry of Labour and Social Security (MLSS). please view the NIS contact details in the “Offices and Contact” section below
  2. Obtain the sealed life certificate from the NIS office or at any NIS office in your parish
  3. Unseal the life certificate by detaching the sealed ends and open the life certificate
  4. Fill out the certificate with your Name, Telephone number, Date, and Signature
  5. After filling the certificate, take it to the either, Justice of the Peace, School principal, Bank manager, Inspector of the police, or a Notary public to authenticate the certificate by signing and stamping it
  6. Then submit the life certificate to the NIS office for processing

Apply by mail

  • Once your life certificate and all supporting information have been completed, signed, stamped, and certified where required, you can submit it to the NIS by mailing the certificate to their address provided in the “Office Locations & Contacts”


Note:

  • If you have changed your address, kindly complete the change of address section and submit it along with the complete life certificate

InstructionsEdit

  • You must sign your Life Certificate
  • Make sure that the person certifying indicates the date of signing.
  • At the point of submission and if the date on your Life Certificate exceeds three months it will be considered invalid.
  • The person certifying the Certificate must affix their stamp or seal.
  • If a civil servant is certifying the Certificate and they do not have access to a stamp, please ensure that they state the Ministry, Department, or Agency to which they are employed as well as their post and rank
  • The Life Certificates are designed with special security features and are only valid for the quarter for which they are generated. Hence, ensure that the quarter ending date corresponds with the quarter for which you are submitting the same.
  • Do not alter the information on the face of the Life Certificate.
  • The barcoded Life Certificate has special features unique to you and the specific quarter

  • The Life Certificate is used to verify that a pensioner is still alive and hence, eligible to receive a pension from the Government of Jamaica.

  • If you don’t submit the life certificate, or you don’t submit it within the required time, or if the life certificate submitted is invalid, payments may be suspended until you provide a valid life certificate to the NIS
