Jamaica - Apply for Funeral Grant Waiver under National Insurance Scheme

Apply In-Person

  1. To waiver your right to seek a funeral grant under the National Insurance Scheme (NIS), you should submit your funeral grant waiver request to the NIS office under the Ministry of Labour and Social Security’s (MLSS) Parish Offices in your location. Please view the NIS parish offices via this link: Link and then MLSS contact details via this link: Link
  2. Obtain the funeral grant waiver request form from the NIS parish office in your location or download it from the MLSS website through this link: Link (scroll to find “Funeral Grant Waiver”)
  3. Complete the waiver request form accordingly in the presence of a witness preferably a Notary Public or a Justice of the Peace official as instructed in the “WITNESS’ CERTIFICATE AND SIGNATURE” section of the form. Note that if the applicant is unable to read or write due to illness or illiteracy, the application can be completed by a recognized witness on the applicant’s behalf
  4. Once the application form has been completed, signed, and authenticated as required, go to the NIS office and submit the complete application to the NIS officer at the service desk
  5. The NIS official will verify your application and issue an acknowledgment receipt of the application
  6. The waiver request will then be submitted to the relevant NIS official for review and processing if the application meets all the required standards of the NIS
  7. The NIS will assess the request and if it is as required, they will register the request in their record and relinquish your right to claim the funeral grant, and then transfer those rights to the relevant individual as mentioned in your application. Note that the said individual must have also applied for the funeral grant

The application form should be witnessed by a recognized official either the Justice of the Peace or a Notary Public office, however, if the application is witnessed by Notary Public official, you should obtain the relevant certificate of commission from the County Clerk’s Office and attach it to the application form
