Apply In-Person

  1. To apply for the Funeral Grant Claim under the National Insurance Scheme (NIS), you should submit your complete application to the department of National Insurance Scheme (NIS), under the Ministry of Labour and Social Security (MLSS) Parish Offices in your location.   2. Obtain Grant’s application form from the NIS parish office in your location or download it from the MLSS website through this link: Link (scroll to find “Funeral Grant Claim”) or you can download the form directly via this link: Link
  3. Complete the application form accordingly and prepare all the required supporting documents and attach them to the application form
  4. Visit the NIS office in your parish and submit the complete application at the service unit
  5. The NIS official will verify your application and issue an acknowledgment receipt of the application
  6. Then the grant request will be forwarded to the relevant NIS authorities for review and processing if the application meets all the required standards of the NIS
  7. Upon processing, the NIS will contact you through a proper communication channel to inform you on how to obtain the Funeral Grant

  1. To apply for the Funeral Grant, you should submit your application to the National Insurance Scheme (NIS) via the MLSS online service portal. Please visit the MLSS website NIS page via this link:Link
  2. Scroll to find the Funeral Grant Applicationand click on the “Apply for Your Funeral Grant Online” button to go to the next page
  3. You will be directed to the funeral grant online application form. Read the provided information carefully and click on “START APPLICATION”
  4. Complete the online form accordingly and upload all the required documents and submit the application
  5. Upon submission, you will be issued a confirmation receipt of your application and you will be required to visit the NIS office to submit the original copies of the required documents
  6. Submit the documents and the NIS will assess your application and approve it for processing
  7. Upon processing the NIS will contact you through a proper communication channel to inform you on how to obtain the Funeral Grant

Required DocumentsEdit

  • The completed Funeral Grant Application Form
  • The Undertakers receipt or invoice
  • Proof of death, such as the Death Certificate, Order for Burial, Medical Certificate of Cause of Death, or Post Mortem Report.
  • Identification document
  • Marriage certificate if applicable
  • All payments after the date of death: These include pension order books with vouchers with payable dates after the date of death, cheques, and electronic payments after the date of death. Note that failure to return all payments after the date of death will result in the sum of these payments being recovered from the Funeral Grant.
