Procedure

Apply In-Person

1. To apply for a delayed claim inquiry under the National Insurance Scheme (NIS), you should submit your application to the NIS office under the Ministry of Labour and Social Security (MLSS). Please view the NIS contact details in the “Office Locations & Contacts” section below
2. Obtain the application form for delayed claim from the NIS office in person or download the form from the MLSS website through this link: Link (choose “Delayed Claim” and download the form)
3. Complete the inquiry part of the application form and write your name and address
4. Visit the NIS office and submit the complete application to the NIS official at the service desk
5. The official will receive the delayed claim application and forward it to the relevant NIS authority for assessment
6. The NIS Division will review your delayed claim request and inform you in writing the reason for the delay
7. Then the NIS will send you the written replythrough your postal address
   

Note:You can opt to submit the complete applicationto the NIS division under the MLSS via mail
