Apply In-Person

  1. Report the incident to the police
    • Report the stolen or loss of your carrier certificate registration to the police station in the area in which you lost the carrier registration certificate, and if you do not remember where exactly, report to the nearest station. You will be issued with a Certificate of loss/police report
  2. Book an appointment
    (i) During the on-going corona virus epidemic period, the Centre de Gestion Intégrée de service(CGI) advises all applicant to make an appointment booking first, via QUIPUX website link link
    (ii) Click on the far right side of your screen, Make appointment option.
    (iii) You will be redirected to the next page, fill out all mandatory fields accurately {location, under formality select “carte de transporteur” , CGI agency that you wish to fill your application with, date, time, email address, full name, phone number, identification document, document number)
    (iv) Accept terms and conditions then click on “To send” tab.
    (v) You will receive details of your appointment, indicating that it has been taken into account by the CGI.
    (vi) If you didn't get any response please check with CGI and get confirmation of the booking by calling the CGI toll free number 1320 or (+225) 21006929.
  3. File your applicant
    (i) Visit the regional integrated Service Management Center (CGI) office, you had book an appointment with at the specific date and time.
    (ii) Approach the Department of transportation service desk and express your interest, fill out the application form accurately.
    (iii) Pick the control ticket service number and wait for your ticket number to be called.
    (iv) Submit the completed form along with the supporting documents (listed below) to the attending official for verification and processing.
    (v) Payment the application fees at the in-house bank, upon which you will be issued with a receipt.
    (vi) If the attending official is satisfied that all the conditions for filling the application are met you the will be issued with the certificate of registration as a carrier within one day.
