Procedure

1. To obtain a Civil Status Identity Card the applicant should have registered his/her marriage with the Civil Status Department.
2. Also, the applicant should have registered his or her birth occurance to the Civil Status Department.
3. A Civil Status Identity request can be submitted to the Civil Status Department who should be able to help you with your query.
