Procedure

1. To have you date of birth corrected on your identification document, you will need to go the secretariat of NOCR Age Ascertainment Commission where your ID card was issued.
2. Submit all the required documents:
   1. Written request in a special form by owner of document or legal guardian
   2. Photo- affixed ID card and its copy (image)
   3. One newly taken photo
   4. The original copy and image of Judgement of Maturity for persons under age 18 who refer personally
   5. The original copy and image of documents relating to guardian and legal administrator for persons under his/ her custody
   6. Payment of charge according to the current tariff
3. The Secretariat will give you a date where they will address your application. You will need to attend the said event set by the Age Ascertainment Commission. If you fail to show up on that stated date, the Commission will make a decision provided that the file was ready for comment; otherwise, your plea is rejected, but you may file another application.
4. If needed the Commission may ask you to take a medical examination to determine your real age.
5. Once the Commission has made a decision, it is final and non-appealable.
6. Execution of the issued decision by Age Ascertainment Commission in identity document and ID card will require written request (by applicant).

Eligibility

You may request for correction of your date of birth, if your real age differs from the recorded age at the identity documents and ID card more than five years.

You may apply for the procedure if you are:

• Eighteen (18) years old or above
• If you are below 18, by presentation of Judgement of Maturity
• Father or grandfather of children under the age of 18, you will need to present your ID card.
• For guardian and legal administrators, you will need to present reliable documents which will verify your position.

Fees
