Indonesia - Report a Damaged Or Lost Certificate

In case of a damaged or lost certificate, you must immediately contact the following:
- The Sub-department for Population and Civil Registration of North Jakarta Municipality, Berdikari #2, North Jakarta, if your certificate was registered at this office and the year of registration is 1997 up to the present day.
- The Sub-department for Population and Civil Registration of West Jakarta Municipality, Meruya (next to MAKRO), if your certificate was registered at this office and the year of registration is 1998 up to the present day.
- The Sub-department for Population and Civil Registration of East Jakarta Municipality, Cipinang Baru Raya #16, East Jakarta, if your certificate was registered at this office and the year of registration is 1996 up to the present day.
- The Sub-department for Population and Civil Registration of Central Jakarta Municipality, Tanah Abang I, Central Jakarta, if your certificate was registered at this office and the year of registration is 1999 up to the present day.
- The Sub-department for Population and Civil Registration of South Jakarta Municipality, Radio V #1, South Jakarta, if your certificate was registered at this office and the year of registration is July 2003up to the present day.
You can also contact The Sub-department for Population and Civil Registration of Jakarta Capital City Government Letjen. S. Parman #7, West Jakarta, if your certificate was registered in a year other than those meant the Sub-departments for Population and Civil Registration stated above.

- Report of Lost Certificate from the local Police Office
- Photocopy of the Family Card (KK)
- Photocopy of Residential Identity Card (KTP)
- Photocopy of Excerpt of Certificate (if there is any)
- completed form
