Procedure

1. Go to the village to report any changes in the family.
2. Submit the required documents stated below.
3. After completing the process, a new family card will be issued.

Required Documents

• Letter RT / RW
• KK long
• Original and Photocopy of Death Certificate / Citation death certificate;
• Original and Photocopy of the Marriage / Divorce those who never married / divorced
• Move Certificate for displaced people, and
• Original and Copy of Deed Divorce citation, for the reduction of members of the family because of divorce.

Note:
Where a whole family moved to another place, then the family card stored in the heads of household must be submitted to the Chairman of RT Headman (repealed). In a new place to live, based on Moving Certificate, Headman will give a new family card.

Processing Time

The processing time for a family card is 14 (fourteen) working days from the date of receipt of complete requirements.

Need for the Document

Family Card contains data on the structure, relationships and family size. Family card must be owned by each family. This card contains data about the identity of the heads of household and family members.

Any changes in the data card due to events such as the Family Birth, Death, removals, etc., heads of household is required to report to the village with the requirements

Information which might help

• Family Card (KK) is a document owned by the provincial government of DKI Jakarta and therefore, it should not be crossed, change, modify, add to the contents of the data included in the Family Card.
• Any changes due to mutation Mutation Data and Biodata, must be reported to the Village and will be issued newly family cards (KK).
• For newcomers who have not registered or have not been a resident of Jakarta, their names and identities should not be included Dalan Family Card.
