Procedure

In United States to a Baptismal Certificate can obtain only through local church where they baptized. Generally, only the church that holds the original records can send a baptismal certificate, and a person can only request a copy of his own certificate to maintain privacy.

Apply In Person:

1. To Get a Copy of a Baptismal Certificate, the applicant has to approach local church or parish where the applicant was baptized.
2. The applicant has to write the request letter to the church regarding the Baptism Certificate.
3. In some church, the application form will be provided to the applicant.
