1. To Correct or Amend a Death Certificate, the applicant has to approach the Indiana Public Health Link
2. The applicant has to contact the respective department officer.
3. Give all the details to the officer to complete the process.
4. If the officer asked for any type documents to submit, the applicant has to submit the documents to the office mail.
5. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
6. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
7. Submit all the necessary documents along with the completed form to the Public Health Mailing address.

10. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
11. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
12. Once the process gets completed, the applicant gets the birth certificate through mail.
13. The processing time for this procedure is up to 15 – 30 days.

• The fee to amend a death certificate is $20.00
• Additional copies of the same record ordered at the same time are $6.00 each.
• There is an additional fee of $15.00 to expedite a request.

• The validity of amendment certificate is valid forever.
