- Application to apply for material permit or license application is available in the following Link
- The applicant has to fill-in the details in the form.
- Applicant should make sure to send the application form along with all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
- Mail the application to the appropriate office through the mail.
- Mailing address is mentioned in “Office Locations & Contacts” of this page.
- The fee of this procedure is mentioned under the “Fees” section of this page.
- Concerned authorities will look into the application form and submitted documents.
- Mail-in applications take four to six weeks to process on average.
- The mailing address will receive notification of the USDOT Number.
- The FMCSA recommends that you keep track of the assigned USDOT number.

- If the applicant is an existing user, they can login directly using their “Username” and “Password”.
- If the applicant is a new user, they have to register first by clicking on the “Create an account” option.
- An applicant has to provide their own email address when they create an account.
- While registering, applicant has to provide all necessary information in the application form and finally click on “Create Account” button.
- Once the registration is successful, the applicant can login with their “Username” and “Password”.
- After logging in, the applicant has to choose the service necessary for them.
- The Applicant must provide all information necessary for the application procedure online.
- The fee of this procedure is mentioned under the “Fees” section of this page.
- If the application is granted, the company receives a USDOT Number immediately after submission.
- The FMCSA recommends that you keep track of the assigned USDOT number.

All facilities or companies that treat, store, or dispose of hazardous wastes now or plan to do so must apply.
FeesEdit
- For the fiscal year 2020–2021, small enterprises and non-profit organisations will pay $250, while everyone else will pay $2,575.
- For each registration statement submitted, all applicants must additionally include a processing fee.
- The processing charge for each registration statement filed for registration years 2000–2001 and later is $25, and a single statement may be filed for one, two, or three registration years.
- The processing charge for each registration statement filed for registration years 1999–2000 and prior is $50, and each statement must be filed separately for each registration year.
