- To Apply for Pradhan Mantri Jan Arogya Yojana, the applicant has to make the application online via link
- If the applicant is a new user, the applicant has to click the “Register” button.
- Enter all the registration details and click the “Submit” button.
- Now, the applicant has to click the “Login” option.
- In the “Sign in” section, please enter the mobile number in the prompted space and click the “Sign In” button.
- Now click on the “Generate OTP” button to receive the OTP.
- An OTP will be sent to the mobile number. Please provide the received OTP and click on the checkbox for the “By clicking submit…” declaration followed by clicking on the “Submit” button to get a new page.
- The applicant has to select the respective state from the dropdown list under Select State. Now select the respective option from the dropdown list under “Select Category” (it is advised to select the “Search By Name” option).
- A new set of prompts will be provided. Please enter the name in the space Name here and complete the other prompts as appropriate. Once all the details are entered/selected, please click on the “Search” button.
- The user will get the “Search Result” page. Here details will be provided based on the “Social Economic and caste census”. Now click on the “Family Details” button for the name that matches the applicant's name.
- A new page with family details will be displayed. Please scroll down and check the details. This page provides two options at the bottom, “Get Details on SMS” and “Information”.
- The user shall select the “Information” option to get the “Guidelines for Beneficiary” page. The page will provide information that is useful for the user. Now it is clear that the beneficiary is eligible to access the impaneled hospital for treatment.
- The “Get Details on SMS” button will be useful to get the details through “SMS” on the respective mobile number. Please use the button and then follow the onscreen information to get the details.
- There is no fee to apply for or be a beneficiary of the Pradhan Mantri Jan Arogya Yojana (PMJAY) in India.
- Now, please visit the nearest CSC center. Provide the “SMS” details as obtained earlier.
- If the application is approved, the applicant will receive a PMJAY card with the family's details and a unique identification number.
- This entire process can be completed within 15 – 30 days or as per the timeline advised by the respective department authority.
Apply through CSC
- To Apply for Pradhan Mantri Jan Arogya Yojana, the applicant has to approach the nearest CSC center – link
NOTE: The applicant has to select state, district, and block name from the drop-down menu, or the applicant has to click on their respective district to find nearby centers.
- Please visit the center and consult with the center operator about your requirements.
- If the operator issues an application form, then collect it and pay the requisite fee. Please complete the application form.
- Now, submit the completed application along with the required documents as per the “Required Documents” section of this page. It is advised to take the documents in their original to the kiosk for scanning purposes.
- The operator will process the application online. On successful submission, the applicant will receive an application number as an acknowledgment. The applicant will also get a transaction number as a text message on their registered mobile number.
- An applicant has to make the necessary payment as the center operator quotes and doesn’t forget to take the receipt.
- This application will be forwarded to the respective department for further processing.
- ”SMS” triggered update on application status will be sent to the applicant’s mobile. Please keep track of the notification.
- The submitted application will be processed by the department. A designated authority (supply officer) will verify the submitted documents.
- There is no fee to apply for or be a beneficiary of the Pradhan Mantri Jan Arogya Yojana (PMJAY) in India.
- Once the process is completed, the applicant has to visit the office to get the card.
- This entire process can be completed within 15 – 30 days or as per the timeline advised by the respective department authority.

- To check the hospital details, the applicant has to use the following link – link
- Enter the state, hospital type, hospital name, and other details.
- After selecting the details enter the captcha code and click the “Search” button.
