Procedure

Apply In-Person

1. The applicant shall visit the district collector office. District Collector or District commissioner is responsible.
2. Refer the office contact details in the “Office contacts & Locations” section below.
3. Collect the application form from the office. Fill in the form with necessary details.
4. Please make sure you have all the documents listed in the “Required Documents” section of this page.
5. After filling the form, attach the documents mentioned in the “Required Documents” section below with the completed application form.
6. Check the application form and then submit it to the concerned authority.
7. Now pay the required application fee to the concerned authority.
8. The officer after validating the application form and the documents will provide you with an acknowledgement/receiving slip.
9. The verification will be done by police and revenue department officers such as Tehsildars, etc
10. After verification, if the authorities are satisfied with the report then the certificate will be processed.
11. Once it is processed, and the certificate is ready, the applicant will receive an SMS or call on the registered mobile number to collect it.
12. The applicant should carry the acknowledgement/receiving slip when going to collect the certificate from the office.
    
    

Apply Online - Apply for Freedom Fighter Certificate Online with Images

1. The applicant shall visit the government website in the below link Link
2. Click on “Login to apply” button , if already a user. Otherwise, click on “Sign up” button.
3. Fill in the required details and click on “Register” button.
4. After signing in, click on “New application”
5. Fill in the mandatory details. Then click on “Next”.
6. Proceed with the page prompt till you fill all the details.
7. Please make sure you have all the documents listed in the “Required Documents” section of this page. Upload the documents whichever is necessary.
8. Click on the “Submit” button. Then click on “Pay” option. The fees can be paid by various payment options available. The confirmation receipt can be downloaded. The confirmation message is also sent through SMS to registered mobile number.
9. The application will be sent to the concerned officer. The officer will verify the details and approve it. The details are verified by Tehsildar
10. After approval, the applicant can find it in the website. The certificate can be obtained online.
    
    

Apply through Citizen Service Centre / Sugam Kendra

1. The applicant shall visit the nearby Citizen Service Centre. Sugam is the Citizen Service Center in the collectorate wherein citizens can avail services.
2. Refer the contact details in the “Office contacts & Locations” section below.
3. Collect the application form from the centre. Fill in the form with necessary details.
4. Please make sure you have all the documents listed in the “Required Documents” section of this page.
5. After filling the form, attach the documents mentioned in the “Required Documents” section below with the completed application form.
6. Check the application form and then submit it to the Sugam operator.
7. The sugam operator will type the necessary details, upload the documents and submit it. Also, pay the required fees amount at the centre.
8. The confirmation receipt will be given to the applicant. The confirmation message is sent through SMS to the registered mobile number.
9. The application will be sent to the concerned officer. The officer will verify the details and approve it. The details are verified by Tehsildar.
10. After approval, the applicant can find it in the website. The certificate can be obtained at the centre.
11. The applicant should carry the confirmation receipt, when going to collect the certificate from the centre.

• Application form for freedom fighter certificate.
• Affidavit by the applicant on non-judicial stamp papers worth Rs. 3/- attested by the Executive Magistrate/Notary (Specimen enclosed).
• Attested photocopy of Tamra-Patra/Pension Payment order issued by the G.O.I. to the freedom fighter.
• Documentary evidence of place of residence at Chandigarh (Voter Card, Telephone/water/electricity bill)
• Copy of date of birth certificate of the children/grandchildren in whom favour the request for issuance of freedom fighter certificate is made.

• Copy of photos to prove that the applicant belongs to the family of freedom fighter.

• Confirmation letter rom Pradhan of the village for the freedom fighter

• The applicant must be a dependant of freedom fighter such as son,daughter,wife, etc
• The applicant should be a resident of the state.
• The freedom fighter who receives the pension from the government

Other uses of the Document/Certificate

The freedom fighters get free railway pass, free medical facilities, telephone connection, reservation under “Combined Category” for Physically Handicapped Personnel(PH), Outstanding Sports Persons(OSP) and Freedom Fighters(FF) in the normal selection procedure adopted by Public Sector Oil Marketing Companies for allotment of petrol pumps, gas agencies etc.
