For those applying for or renewing their Hawaii State Identification Card, you must present documentary proof of legal name, date of birth, social security number, legal presence and proof of principal residence address in person at any of the following Driver License locations:

Note: Documentary proof affects anyone applying for a Hawaii State Identification Card, including U.S. citizens and foreign-born applicants. The documentary proof also applies to anyone who has just moved to Hawaii from another state or country, and anyone renewing their Hawaii State Identification Card.

Required Documents

Documentary proof documents: U.S. birth certificates and U.S. passports.

Foreign born applicants may present other documents, such as: Certificate of Citizenship or Naturalization, Resident Alien Card, or a valid foreign passport with a Visa and I-94.

Documents presented for proof of name, date of birth, legal presence and social security number must be valid originals or certified copies.

Photo copies or fax copies of proof documents are not acceptable.

Eligibility

Applicant must be a minimum age of ten years old.

For applicants who are under the age of fourteen years: the application shall be made on the minors behalf by the parent, or by another individual in loco parentis of the minor who can provide proof of guardianship. In the case of an incapacitated/incompetent individual, the application shall be made by the individual having custody or control of or maintaining the incapacitated/incompetent individual as indicated in appropriate documentation.

Fees

• New - 1 Year: $5.00
• New - 8 Year: $40.00
• Renewal - 1 Year: $5.00
• Renewal - 8 Year: $40.00
• Duplicate: $6.00

Validity

• For Residents: The card expires on the eighth birthday after the year of issuance
• For Legal Non-Immigrants: The card expires on the same date as the expiration of legal stay.

Instructions

Each individual applying for an initial Hawaii State Identification Card will be required to provide documentary proof of:

• Legal name and date of birth
• Legal Presence
• Social Security Number
• Proof of Principal Residence (Two documents required)

A State Identification Card can be availed by any citizen of this State, young or old alike. This card is a good example of a proof of identification.

Other uses of the Document/Certificate

This identification card is an alternative to a drivers license. Though it is not prohibited to have both, yet these program is initiated to cater those part of the populace that does not know how to drive a vehicle.
