Procedure

One has to make police complaint and obtain a FIR for applying duplicate copy in case of the permit is lost or stolen.
If the permit is damaged, torn or mutilated then, the remains can be submitted for applying duplicate permit

Apply In-Person

1. To apply for duplicate Private Service Vehicle Permit in Haryana the applicant has to approach the RTO office to which his area belongs to.

3. Application in white A4 sheet
   
4. Please make sure you have all the documents that are listed down the “Required Document” section of this page.
   
5. Present the completed application forms in one of the counter and pay the appropriate fee where the officials would start processing your application.
   
6. After checking the vehicle for its correctness and other appropriate requirements the application will be processed. Important verifications for the permit includes Vehicle condition, Meter for fare, seating arrangements etc., will be carried by authorities.

Apply Online

1. Use the following link : Parivahan Home page and select “Permit Online” option under Other online services
   
2. Select Haryana in the drop down menu and click “proceed”
   
3. Please follow the screen and fill in prompted details to process your online procedure
   
4. Upload required documents and take printout of acknowledgement.
   
5. On the appointment date, go to RTO office before 11:30 am and submit the documents. Once you submit and pay the fee, rest of the procedure will happen as per how you apply in person.

2. Enter the application number as prompted and click “Get user Details” to process online in getting appointment

3. Defaced/Mutilated/Completely written off permit.
   
4. Pollution under control certificate
   
5. Copy of Registration Certificate of the vehicle.
   
6. Copy of Fitness Certificate of the vehicle.
   
7. Copy of Insurance Certificate of the vehicle.
   
8. Payment of green tax wherever applicable.
   
9. Aadhaar card

1. Private Service Vehicle Permits are issued to institutions that operate the vehicle for their own use.
   
2. Whenever such permit is lost or damaged or missing, then the Duplicate for the permit has to be applied
   
3. An application for a permit to use a motor vehicle as a private service vehicle shall contain the following particulars, namely:-
   

• Type and seating capacity of the vehicle
  
• The area or the route or routes to which the application relates
  
• the manner in which it is claimed that the purpose of carrying persons
  
  

Information which might help

• One needs to provide the declaration to use the vehicle only for faring employees or laborers of a Company/Firm/Institute to ferry them to the place of business or back.
  
• Disproportionate asset case should not be pending against the vehicle for issue of permit.
