Gujarat - Correct or Update a Birth Certificate

Apply In-Person:
- Applicant shall approach the Gram Panchayat, Taluka Panchayat, & Nagarpalika Offices.

- Obtain 'Birth Certificate Update/correction Form' from the respective counter or write in a paper OR use the following link to download the form to apply. link
- Please complete the application form and attach the required documents and submit it to the receiving authority
- The accepted application and documents will be verified by the authority and a acknowledgement will be provided for applying.
- Collect the acknowledgment slip after submitting the form and keep it safe for future reference.
- This application will be processed further by the respective department.
- Once the process is over, applicant gets notification to collect the certificate.
- Use the acknowledgment slip to obtain certificate from the office as per notified period.
- Note: This changes will be published in a local newspaper and with the respective state official Gazette publication.

- Affidavit from local notary
- Affidavit on a non-judicial stamp paper of minimal about the reason of name change.
- Address proof (Voter ID, Driving License etc.) of parents as per requirement
- Aadhaar card

- Date of birth of the child
- Location of residence
