Apply In-Person
- Ration Card holder should approach The Mamlatdar office /Jan seva Kendra to add or delete name in ration card.
- Contact link1 in the given page, please find the “District” and Zone/Taluka” prompts under “For the customer” section. please select the district name and zone/taluka as appropriate for contact details AND link2 please use the contact details to get the contact details of the respective office to apply AND link3 in the given page, please select the website hyperlink as per your choice to reach respective district website. Please select “Offices > Mamlatdar Office” menu to get mamlatdar contact details and “HOME > SERVICES > Digital Gujarat” or “DEPARTMENTS > COLLECTORATE-REVENUE > Registry (Jan Seva)” for Jan seva Kendra at respective municipality.
- Please go to the respective office.
- Applicant shall obtain the form from the respective office (please pay for the form if required) Or a written/electronically typed application on A4 plain sheet as advised shall be used OR use the following link for application form listed. Link: form link
- Please submit the completed application along with required documents to the respective authority.
- Authorities after checking the submitted application along with attached documents will make entries and provide you record number for reference.
- Keep it safe for future reference.
- This application will be forwarded to the concerned Ward/ Grama Panchayath official for verification. The concerned official visits the house of the applicant for verification and approves the request, if the applicant is found eligible.
- The applicant is informed through SMS at various stages i.e., Advance intimation about date of visit of the official to his place of residence for verification of his application, Approval/Rejection of his application.
- After ground level verification ration card will be issued in 2 – 3 weeks or as notified by the department

- Please hit the “Addition of Name in Ration Card” tab or “Removal of Name from Ration Card” from the list of services provided to reach next page.
- In the given page, please go through the details and hit the apply online option provided at the bottom.
- In the given page, user need to select email/mobile from the drop down for login.
- Complete the page prompt to login by clicking login.
- If you are a new user to this portal, then click on “Click For New Registration (Citizen)” link, it will redirect you to user registration page to register
- Once login, user need to hit the respective option and complete the successive page prompts and also upload required documents to apply online by hitting the right tab.
- After successful submission of your application you can take the print of your application form and proceed further with the online payment.

- This application will be processed by the respective department.
- Citizen will get an SMS for the status of his/her application and has to follow as per notification to get the card.
- Note:

- If your application is returned for change or to fill incomplete details, kindly submit it within 37 days of return. If fail to submit within 37 days application will be disposed with rejection. Application fee will not be refunded.
Apply through CSC:
- To Add or Delete Name in Ration Card, the applicant has to approach the respective “CSC” center link
- The applicant has to select state, district, and block name from the drop-down menu, or the applicant has to click on their respective district to find nearby centers.
- Please visit the center and consult with the center operator about your requirement.
- If the operator issues an application form, then collect it and pay the requisite fee. Please complete the application form.
- Now, submit the completed application along with the required documents as per the “Required Documents” section of this page. It is advised to take the documents in their original to the kiosk for scanning purposes.
- The operator will process the application online. On successful submission, the applicant will receive an application number as an acknowledgment. The applicant will also get a transaction number as a text message on their registered mobile number.
- An applicant has to make the necessary payment as the center operator quotes and doesn’t forget to take the receipt.
- This application will be forwarded to the respective department for further processing.
- ”SMS” triggered update on application status will be sent to the applicant’s mobile. Please keep track of the notification.
- The submitted application will be processed by the department. A designated authority (supply officer) will verify the submitted documents and inspect the area of the applicant to clarify the applicant’s details.
- Based on the outcome of the above-mentioned steps and other internal processing, the designated issuing authority will decide on the issuance of the new ration card.
- The status, viz. Approval or Rejection will be conveyed to the applicant through an “SMS” or applicable norms.

- A valid ration card holder.
- The applicant and his/her family must be Indian citizens and should be resident of the state.
- The applicant or his family members must not possess any family card in any State in India (If they had one, it has to be withdrawn).
- The applicant and members of the family must be close relatives.
