Procedure

How to Apply:
In-Person Application:

1. Visit the Food and Drugs Authority (FDA) regional office near you Food division, Animal Product Department (APD) and express your interest on the service desk.
2. Purchase the prescribed Application forms at a cost at the FDA office’s or downloaded via the FDA website portal website or click the link below to download (Application form)
3. Submit the completed forms along with the supporting documents (listed below) at the APD office for verification and processing.
4. Pay the processing fees, upon which you will be issued with a receipt.Note that the fee is non- refundable.
5. If the authorized official is satisfied that all the conditions are meet as required for the application, you will be notified of the outcome and issued with the licences upon payment of the required renewal fees.

• Citizens need to be in possession of a valid national identity card.
• Companies need to be in possession of a valid company's registration certificate.
• Road worthy certificate
• Copy of the expired licence
• Valid Driver’s license
• Valid Vehicle insurance certificate
• Thermometer calibration certificate
• Temperature log book/chart
• Training records – basic food hygiene & cold chain management
• Vehicle maintenance schedule and records – cleaning & preventive maintenance of freezing system.
• Pest Management schedule and records.
• Licensing Renewal fee as stated in the Food and Drugs Authority’s fee schedule
• Any other documents that may be requested by the FDA.

• The Food and Drugs Authority (FDA), formerly, Food and Drugs Board (FDB) is the regulatory authority with the responsibility of implementing the Food and Drugs Law of 1992, (PNDCL 305b). Part seven of the Public Health Act, 2012, Act 851 mandates the Food and Drugs Authority (FDA) to protect and promote public health by ensuring that food and drugs consumed in Ghana are wholesome and safe.

Need for the Document

• Food and Drugs Authority (FDA) and the Veterinary Services Directorate (VSD) by Public Health Act, 2012 (Act 851) requires that all meat and fish transportation vehicles be registered with the FDA and obtain a licences after meeting the acceptable standards i.e. Good Cold Storage, handing and transportation Practice as set by the FDA, before commencing operation.
• These is to ensure the safety and quality of meat for human and animal consumption in Ghana as the Codes of Practices
• Such Companies/business/person that are registered with the Food and Drugs Authority (FDA) must renew their licence to keep enjoying the incentives that come with registration with FDA.
