Apply In-Person:
- To Register a Death and Obtain Certificate in Ghana, the applicant has to approach the ADA East District Assembly office or Birth and Death Registry Headquarters office.

- To apply for a Death Certificate, the applicant must first register the Death.
- The death has to be registered with the concerned local authorities within 21 days of its occurrence, by filling up the form prescribed by the registering officer.
- As a first step a death should be notified to the respective authorities at the earliest it occurs.
- Once the information is registered / reaches the Birth and Death Registry office, the concerned citizen has to approach the authority and apply for the certificate on any working day.
- Applicants can get their application form from the relevant department.
- Applicant has to enter all the information in the application form.
- Applicant must make sure to carry all the necessary documents that are mentioned under “Required Documents” section of this page. (Both photocopies and originals)
- Applicant should attach photocopies of documents (whichever is necessary) along with the applicant from while submitting it at the office.
- Applicant will be required to pay the appropriate inspection fees and submit the payment receipt. The amount of fees to be paid can be found under the "Fees" section of this page.
- Submit the completed application form, required documents.
- If all the above mentioned procedures were followed properly by the applicant, the application form will be viewed by concerned authorities and the applicant will be notified regarding their certificate.
- This application will be processed and information will be verified by the BDR office.
- If the verification is successful, notification will be sent to the applicant.
- Applicant shall collect the death certificate as per notified period (usually on same day).
- The processing time for this procedure takes 10 days.

The death certificate is important for three basic reasons:
- Legal Reasons: The death certificate is a permanent legal record of the fact of death. Oklahoma law stipulates that a death certificate is to be filed (63 OS 1-317(a)). Therefore, it is a legal requirement. It provides important information about: the decedent, the cause of death, and final disposition. This information is used in the application for insurance benefits, settlement of pension claims, and transfer of title of real and personal property. The certificate is prima facie evidence of the fact of death and, therefore, can be introduced in court as evidence when a question about the death arises.
- Personal Reasons: The death certificate in many cases provides family members with closure, peace of mind, and documentation of the cause of death. It also provides peace of mind by facilitating efficient processing of needed benefits as those described above.
- Vital Statistics Reasons: The death certificate is the source for state and national mortality statistics. It is needed for a variety of medical and health-related research efforts. It is used to determine which medical conditions receive research and development funding, to set public health goals and policies, and to measure health status at local, state, national, and international levels. This data is valuable as a research tool and by influencing research funding.
