Apply In-Person:
- To Correct or Update (Information, Address) on Birth Certificate, the applicant must go to the Register General Department.
- Address and contact details can be found in the following Contact link
- Information related to the AMA office and the License authority can be obtained from the “Office locations and contacts” section of this page.
- Applicant must obtain the application form from the relevant department.
- Applicant has to enter all the information in the application form.
- The applicant has to make sure that he or she has all the required documents that are required for this process to be successful and they can be found under the "Required Documents" section of this page.
- A fee for this procedure has to be paid as per the authority’s request.
- The completed application must submit to the respective office.
- The accepted application and documents will be verified by the authority and an acknowledgement will be provided for applying.
- Collect the acknowledgment slip after submitting the form and keep it safe for future reference.
- This application will be processed further by the respective department.
- Once the process is over, applicant gets notification to collect the correct or update on birth certificate.
- Use the acknowledgment slip to obtain certificate from the office as per notified period within 15 days.

- Affidavit on a non-judicial stamp paper of minimal about the reason of name change.
- Address proof (Voter ID, Driving License etc.) of parents as per requirement
