Procedure

Apply In-Person:

1. Application form for a Food Hygiene Permit is available in the Food and drugs authority(FDA) regional office in Accra contact link
2. The applicant has to approach the FDA office. Then, the applicants can get their application form or click the link below to download. link.
3. Read carefully through the application forms as there contain different section.
4. Please complete each section of this application form as a Word document then submit a printed version of the completed form, along with the supporting documents to the FDA regional office or the head office for verification and processing.
5. Applicant must make sure to carry both photocopies and originals of all documents are necessary for completing this procedure.
6. Please refer to the fees details in the “Fees” section of this page.
7. The FDA will dispatch an inspector to the facility premises for inspection to determine if Good Manufacturing Practice (GMP) and Good Storage Practice (GSP) during the packing, processing, storage, labelling, sterilizing, testing, dispatch, quality control, and quality assurance of the products.
8. A copy of the report finding will be given to the applicant and if any modification or upgrading is required, Corrective Action and Preventive Action Report (CAPA) report will be issued and you will be required rectify the problem.
9. Upon successful implementation of the CAPA, a follow-up inspection may be conducted by the inspection team to ascertain the effectiveness of the implementation (where applicable).
10. If the Expert committee formed by the FDA are satisfied that all the conditions are meet as required for the application, the applicant will be notified of the outcome and issued with the license by the register/authorized official.
11. Generally, the processing time for this procedure is 1 month.

Instructions

• FDA can reject you application and you have the right to address any issue raised concerning their applications within a period of three (3) months of receipt of the notice.
• If the FDA does not receive any response within the period specified you will reapply for licensing.
• An appeal for the review of an application may be made in writing to the Minister of Health within thirty days of receipt of the rejection notice.

Need for the Document

• Establishment that operate with food preparation services are required to register and obtain Food hygiene permit from the Food and Drugs Authority’s (FDA) after meeting the acceptable Good Manufacturing Practices (cGMPs) that promote safe food processing environment in relation to Part Seven, Section 130 & 131 of the Public Health Act, 2012, Act 851.
