Procedure

Apply In-Person:

1. To Apply for Birth license in Ghana, the applicant has to approach the ADA East District Assembly office or Birth and Death Registry Headquarters office.

3. Applicants can get their application form from the relevant department.
4. Applicant has to enter all the information in the application form.
5. Applicant must make sure to carry all the necessary documents that are mentioned under “Required Documents” section of this page. (Both photocopies and originals)
6. Applicant should attach photocopies of documents (whichever is necessary) along with the applicant from while submitting it at the office.
7. Applicant will be required to pay the appropriate inspection fees and submit the payment receipt. The amount of fees to be paid can be found under the "Fees" section of this page.
8. If all the above mentioned procedures were followed properly by the applicant, the application form will be viewed by concerned authorities and the applicant will be notified regarding their license.
9. This application will be processed and information will be verified by the AEDA office.
10. If the verification is successful, notification will be sent to the applicant.
11. After seeing this notification, the applicant must go to the respective office and the applicant can get their license.
12. Applicant’s birth certificate will be ready after 14 working days.

Eligibility

• Any citizen and foreigner who is born in the Ghanaian city can apply for this application.

Fees

• It cost GH₵50.00 to apply for the Ghanaian birth license.

Validity

• Birth License Valid Forever.

• The name of the new born baby.
• Date and the place of the birth.
• The sex of the child (Whether male or female)
• The type of birth (i.e. whether single or multiple as in twins/triplets etc.)
• Exact place of Birth / Name of hospital where child was born
• The nature of birth.
• Birth Notification No.
• The particulars of the mother. I.e. the name, age, marital status, the previous births and the area/district the mother resides in.
• Full Name of Mother before marriage
• The name of the father for the married couples.
• Applicants' Address
• Postal Address of Child (For late application)
• Name of hospital and address
• Residential address at the time of birth

• Birth license is the most important identity document that makes it possible for anyone in possession of it to benefit from a gamut of services offered by the Ghana government to its citizens.
