Procedure

How to Apply:
In-Person Application:

1. Visit the Food and Drugs Authority (FDA) regional office or head office in Accra, Food division, Agro Produce and Biosafety Department (APBD) and express your interest on the service desk.
2. Purchase the Application forms at a cost at the FDA office’s or downloaded via the FDA website portal Website or click the link below to download (Word doc application form)
3. Read carefully through the application forms as there contain different section.
4. Please complete each section of this application form as a Word document then submit aprintedversion of the completed form, along with the supporting documents (listed below) to the FDA regional office or the head office for verification and processing.
5. Pay the processing fees, upon which you will be issued with a receipt.Note that the fee is non- refundable.
6. The FDA will dispatch an inspector to thestorage facilities for inspection to determine ifGood Cold Storage Practices (GCSPs), Good Handling Practices and Good Manufacturing Practices are meet.
7. A copy of the report finding will be given to the applicant and if any modification or upgrading is required,Corrective Action and Preventive Action Report (CAPA) report will be issued and you will be required rectify the problem.
8. Upon successful implementation of the CAPA, a follow-up inspection may be conducted by the inspection team to ascertain the effectiveness of the implementation (where applicable).
9. If the authorized official is satisfied that all the conditions are meet as required for the application, you will be notified of the outcome and issued with the licences upon payment of the required fees.

• Citizens need to be in possession of a valid national identity card copy.
• Companies need to be in possession of a valid company's registration certificate copy.
• Copy of most recent manufacturing premises license if facility has previously been licensed.
• License fee as stated in the Food and Drugs Authority’s fee schedule.
• Sketch directions to the processing facility, indicating landmarks, if any
• Any other documents that may be requested by the FDA

Note :

• A copy of the submitted documents shall be endorsed by the FDA and returned to the applicant.

Need for the Document

• Agro produce storage (warehouse and cold room) operators are required to register and obtain licenceof processing facilities from the Food and Drugs Authority’s (FDA) after meeting the acceptable standards as set by the FDA.
• The aim of these is to maintain the integrity of the produce for their intended purpose in relation to the Public Health Act, 2012, Act 851.
