Georgia - Obtain Death Certificate

- Submit your request to a obtain a copy of the death certificate at any branch of Public Service Development Agency or Public Service Hall. (contact information provided below).
- Do not forget to bring a valid identification. The registry personnel may ask for your ID in order to confirm your identity and make sure that you have the right to obtain the certificate.
- When you submit your request make sure that you have provided these important details which will make searching for the record easier:
- Name of the deceased
- Sex
- Date of death#Pay the application fee.

- Certificate of death is issued to any person
- Civil acts registration certificate drawn up in the name of a deceased person is issued to his/her heirs
- Civil acts registration certificate may be issued to any person on the basis of notarized power of attorney
