Procedure

1. Submit your request to a obtain a copy of the death certificate at any branch of Public Service Development Agency or Public Service Hall. (contact information provided below).
2. Do not forget to bring a valid identification. The registry personnel may ask for your ID in order to confirm your identity and make sure that you have the right to obtain the certificate.
3. When you submit your request make sure that you have provided these important details which will make searching for the record easier:
   1. Name of the deceased
   2. Sex
   3. Date of death#Pay the application fee.

• Certificate of death is issued to any person
• Civil acts registration certificate drawn up in the name of a deceased person is issued to his/her heirs
• Civil acts registration certificate may be issued to any person on the basis of notarized power of attorney

Fees

• Fifth business day: 2 GEL
• Third business day: 7 GEL
• Second business day: 17 GEL
• The same day: 27 GEL
