If you want to change the name or surname you may refer to the Public Service Hall.
For the agency to consider your case, you need to submit following documents:
- An application to change the name and/or surname;
- Identity document;
- Birth certificate (a copy);
- If married - marriage certificate (a copy);
- If you have a minor - birth certificate (a copy);
- If an application is launched by a minor between the age of 16 to18 - concept of parents, or person, performing parental liabilities,;
- If the applicant wants to have the surname of the actual custodian or a relative of the direct ascending line, documents certifying the respective fact.

EligibilityEdit
You may simply wish to change the name. As for the surname, you may want to :
- join spouse's surname to your surname;
- to adopt the surname of an actual custodian;
- to return your premarital surname;
- to get the surname of your relative in the direct ascending branch of the family name (including mother, father, grandmother, grandfather, etc.);
- to receive the surname of your adoptive parent;
- to adopt your parents' joint surname.

If you are a suspect of a crime or wanted by the law enforcement agencies, you might be denied the right to change your name and/or surname.
You may wish to change your name for one of many reasons - you may have gotten married or divorced, for example, or you may simply dislike your birth name.

Need for the DocumentEdit
If you are using, or wish to use, a name other than your registered name, there is no legal requirement that you formally change your name. It is, however, recommended that you change your name officially, both for identification and as evidence of your name change.
Name change generally refers to the legal act by a person of adopting a name different from their name at birth, marriage or adoption. The procedures and ease of a name change vary between jurisdictions.

Refugees and IPDs, upon submitting document confirming their status, are exempt from the state duty.
In cases of name/ surname change, you are obliged to replace identification documents.
Additional requirements:
If you are a minor under 16 years of age, your name and surname can be changed upon the will of your parent (parents) or a custodian. If you already reached the age of 10, together with the will of your parent or custodian, your written consent is required.
If you are a minor under 16 years of age, the decision to change your name or surname is reached through an agreement of both parents, in case of disagreement - the court makes decision, taking into consideration you best interest.
If you are a minor between 16-18 years of age, for changing name and/or surname, you need a concept of your parents or a legal custodian.

Public Service Hall is the building where most of public services are presented. Its purpose is to issue documents and render services the exclusive right to which is owned by State. It is impossible to get such important and necessary documents as a birth certificate and identity card, passport or marriage certificate from any other part other than the State. Accordingly, the State has an obligation to simplify significantly the relationship of citizens with the public sector through prompt and effective steps.
