How to Apply:
Step 1: Obtain affidavit and police abstract:
- You should start by visiting a police station to report the loss/damage of the passport immediately. You are also to acquire a police abstract from the police authorities which is a prerequisite to making the application.
- You are also to swear an affidavit of the circumstances surrounding the loss, theft, mutilation or damage. You are to attach this affidavit with your application.
Step 2: Make the application:
- Visit the Directorate General for Documentation and Immigration (DGDI) office near you and express your interest at the service desk.
- You will be required to re-apply for the passport again, Application form can be obtained from the DGDI office, or download it via the DGDI website portal or click the link below to download,:(Application form)
- Submit the completed duly filed application form along with the supporting documents (listed under “Required Documents” section) for verification and processing to the attending official at the DGDI.
- If the authorized official is satisfied that all the documents are in order, the Information obtained from the application form will be entered into the DGDI data system.
- Pay the required application fees.
- Your biometric information will be taken on site and you will be issued with an acknowledgement/receipt slip and you will be notified of the outcome and when to came and collect the passport.
Abroad Application :
- In case you have lost/stolen/mutilated your passport, you should visit the Gabon embassy or consulate near you with the police affidavit/report and a letter explaining how you lost the passport.
- Will be issued with a travel document to help you travel to Gabon for passport application.

Required DocumentsEdit
- An affidavit of the circumstances surrounding the loss, theft, mutilation or damage.
- A police report on the loss of the passport. i.e. Police abstract.
- Letter explaining how you lost the passport
- One recent color photo
- Copy of the lost passport (if available)
- A legalized copy of the birth certificate as well as the decree and the nationality certificate for naturalized persons
- Parental authorization for children under 16 and copy of the identity document of the parent who signs the authorization
- A receipt from the public treasury of 5,000 Frs cfa (to be withdrawn from the treasury)
- Proof of resources (certificate of employment or care)
- Any supporting document relating to a possible change of name or identity
